This achievement is unlocked by killing Oculus the Inflamed, a foulspawn boss from the Spellplague Caverns. The creature was part of the Ending the Madness quest given by Quorthon's Astral Form deep in 3rd zone of The Chasm. It was a dungeon encounter that has since been removed from the game. Because the dungeon has been removed Oculus has been moved to a side passage at the top of The Jagged Approach, right before you enter the caverns for the Source of Corruption quest. Take the passage to find Oculus, kill the creature, and the achievement will be yours.
