To unlock this achievement you simply need to collect 250 pieces of lore throughout the game. A piece of lore is considered information about places, people, and things in the many zones/regions found in the game. They appear on screen when you discover something new about the game. You may then access the information when prompted or you can review the information anytime from the Quest/Journal/Lore menu path. There are more then 250 pieces of lore in the game and you should unlock this achievement naturally if you explore the majority of the game and the many expansions.
