Right where you get unicorn food achievement, go behind the salesman's cart and there is a path. This path takes you to the river you have to cross, into a cave the river runs through and when you follow this all the way to the back (dead end) you will earn this. Most items in this game have a significance, but I don't know the reference here. Maybe it comes up in a later chapter.
