Disclaimer: Complete "Playing Doctor Achievement" before starting any missions at the beginning of the game in Portland. It will become extremely difficult once gang members become hostile.

The easiest place to do this is Portland, and you can do it early in the game. I suggest you get to know the map a bit before you try, though. You can find a fire truck near the Junkyard. Get in and press up on the d-pad to start the mission.

It’s not that difficult. Just remember to use shortcuts, every fire you distinguish resets the time. Press LB to spray water and use the RS to aim.
