Way easier than it sounds. Load up Lone Wolf, the final mission in Reach. Make sure you’re playing on Legendary, and have 3 scoring skulls (ones that add a multiplier) activated.

Now all you had to do is die, thereby completing the mission. Don’t let the rarity fool you, it’s that easy.
