According to the official Xbox Japan Blog, it was too difficult to get the Ultimate Edition, with its new content, to conform to Japanese ratings regulations. Previous Gears of Wars titles, however, were released in Japan.
The versions of the previous games had modified releases in Japan. Many of the visuals and animations were changed to conform to ratings regulations and cultural differences. That's why there are separate, stackable JP versions.

Makes sense that they would not devote the resources to completely remake the game a second time for the JP market. There likely isn't enough demand to cover the costs.

Although I'm still not sure how or why Japan would not allow the imagery of Gears of War yet still allow things like Urotsukidoji: Legend of the Overfiend... and the such.

Microsoft has always found it hard to penetrate the Japanese market and something like this puts even more obstacles in front of them. It may even impact on the release of Gears of War 4 next year too.

This is why Microsoft is looking towards China recently instead. China has recently lifted its ban on video game consoles, so the Xbox One could fill that void.

China has over 1 billion people so any fraction of that market will be big. Some Hollywood movies have benefited a lot releasing in China such as this year’s Furious 7 and more.

For any Japanese gamer out there, they might have to import Gears of War Ultimate Edition overseas in order to play it.
