I think it's just your whole profile that get's scanned and I believe it has multiple scanners to lessen the load of the site, So say early morning you 360 achievements may get synced then later in the day the One achievements, then later the rest of the odds and ins on your profile, OR 3/4 of the site gets; synced then later another 3/4 and so fourth until everyone is synched. Premium members get synched more often.

If your missing achievements or games played sometimes just replaying that particular game may bring that up closer to sync. If you do not have said game anymore we maybe able to manually synch your data. This is also all dependent on your privacy settings on your account.

The real process is kept under tight security by our devs... but if your having issues I'm sure we can help you out, we just need the specifics.
