After you defeat the main story's Rotten Apple Cat boss, it will drop an apple cat to pick up. Pick this up.

Take this back to the lady in the bottom left of starting area of the game. She's literally next to other apple cats. For returning her cat, she will give you a like fanart waifu drawing of yourself.
