Use a revive token.

I picked up my first Revive Token sometime early on in either the Ego or Paranoia Layers. Once you lose all your health you have the option to use a Revive token or Quit. Simply use the revive token you've found and the achievement will pop. You will also occasionally be given revive tokens by the subconscious as you progress through the game.
